Are you a passionate and detail-oriented Clinical Pharmacist ready to make a significant impact on patient health? Responsibilities:
- Conduct comprehensive medication reviews (CMRs) and deliver in-depth consultations, focusing on critical medication use according to established clinical guidelines.
- Engage patients through outbound phone calls, performing a high volume of consultations daily to ensure widespread impact.
- Accurately transcribe patient-specific prescription and diagnosis information, along with your clinical recommendations, into a web-based application.
- Contribute to the creation of standardized patient documents, ensuring clarity and accuracy in communication.
- Actively participate in the medication therapy management (MTM) product team, contributing to program development and improvement.
- Attend training sessions to continuously enhance your skills and knowledge, staying at the cutting edge of clinical pharmacy practice.
Must-Have Qualifications:
- Valid Pharmacist License in Florida.
- Minimum of 4 years of experience as a clinical Medication Therapy Management (MTM) pharmacist.
- Familiarity with driving key quality measures related to medication use and patient safety.
- Strong understanding of pharmaceutical and medical vocabulary.
- Demonstrated clinical experience in medication review.
- Fluency in speaking, reading, and writing English.
- Excellent verbal and written communication skills.
- Proven tech-savviness, comfortable with various digital tools and platforms.
- Self-directed, organized, and detail-oriented with a strong sense of urgency.
- Ability to maintain discretion and confidentiality with sensitive patient information.
Nice-to-Have Qualifications:
- Management experience within a clinical MTM setting.
- Experience using CRM software.
The target hiring compensation range for this role is $50 to $57 an hour. Compensation is based on several factors including, but not limited to education, relevant work experience, relevant certifications, and location.
