Are you a dedicated Clinical Pharmacist ready to make a tangible difference in patient health? This pivotal role offers you the chance to directly shape medication outcomes, providing essential therapy management and in-depth consultations. You’ll be instrumental in ensuring patients understand and benefit from their medication regimens, contributing to improved health and well-being on a daily basis.

To be considered for this role, you must:

  • Be authorized to work in the United States
  • Not require sponsorship of any kind for the duration of the assignment
  • Be able to work on a W-2 basis.  C2C or 1099 is not permitted for this position
  • Have a current and valid Pharmacist’s License for the State of Florida

Role and Impact

In this role, you will be a key member of the medication therapy management team, driving critical initiatives to enhance patient safety and medication effectiveness. Your expertise will directly influence patient education and adherence, transforming complex medical information into actionable insights for individuals. You’ll play a vital part in a system designed to deliver personalized care, impacting quality metrics and patient satisfaction.

Key Responsibilities

  • Engaging with patients through outbound calls to provide personalized medication support, with an expectation of approximately 100 calls per day.
  • Performing comprehensive medication reviews.
  • Delivering medication therapy management services.
  • Accurately and efficiently transcribing patient-specific prescription and diagnosis information, along with recommendations, into a web-based application to generate personalized patient documents.
  • Participating in continuous learning and training sessions to enhance skills and stay current with best practices.

Must-Have Qualifications

  • A valid Pharmacist License in Florida.
  • At least 4 years of experience as a clinical professional specializing in medication therapy management.
  • Familiarity with key quality measures related to medication use and patient safety.
  • A strong understanding of pharmaceutical and medical vocabulary.
  • Demonstrated clinical experience in medication review processes.
  • Fluency in spoken and written English.
  • Excellent verbal and written communication skills.
  • Proficiency with technology and various software applications.
  • Self-directed and highly organized.
  • Ability to maintain strict confidentiality and discretion.
  • Exceptional attention to detail.
  • A proactive approach and a strong sense of urgency.

Nice-to-Have Qualifications

  • Management experience.
  • Experience with customer relationship management (CRM) software.
